Many guides promise a $1,000 prize, a 7-day deadline and even coupons, but most of that can’t be traced to anything from CVS. This guide sticks to what we could actually check: the survey itself, a dated CVS receipt, and CVS’s own contact pages. Start with the checker to make sure you have the right number.

Where to Find the Survey ID on Your CVS Receipt
On a real CVS receipt dated May 28, 2026, published by Tripl, the Survey ID is printed near the top, right after the list of items, under the heading “Survey ID #”. It’s grouped like this:

How to Take the CVS Health Survey
- Type www.cvshealthsurvey.com into your browser’s address bar. Searching for it mostly shows unofficial sites, so typing it is the reliable way.
- You’ll be forwarded to the CVS Retail Customer Experience Survey on survey.medallia.com.
- Choose English or Español from the “Choose Language / Elija el idioma” menu.
- Enter your 17-digit Survey ID exactly as printed, then select Next.
- Answer the questions about your visit honestly, and finish to the end.
Is There Really a $1,000 CVS Survey Prize?
We couldn’t confirm one. The first survey screen thanks you for shopping and asks for your Survey ID, with no mention of a prize, a drawing or official rules, and we found no current CVS sweepstakes rules anywhere online.
Some older guides quote rules for a “CVS Pharmacy $1,000 Monthly Sweepstakes” from several years ago. CVS doesn’t publish those rules today, and guides now disagree on whether the prize is cash or a gift card, how many winners there are, and where to mail entries. That’s why we don’t list a prize, a mailing address or entry limits.

- Survey ID rejected: use the checker above. It should be exactly 17 digits, not the 18-digit barcode number.
- Letters in the ID: the Survey ID is numbers only, so watch for O instead of 0, and I instead of 1.
- Old receipt: CVS doesn’t publish a deadline, but survey IDs may stop working after a while. Try a recent receipt.
- Page won’t load: try another browser, a private window, or switch between Wi-Fi and mobile data.
